Printable field packet
A neighborhood inventory people can actually use.
Use the first two sheets on a quiet day. Keep the last two blank until something happens. Paper isn't glamorous, but it doesn't ask for a password while the power is out.
This page doesn't collect or save anything. Print it or copy the headings by hand. Participation should be voluntary, and private information should stay with the people who need it.
Place, routes, and public sources
Draw the area people mean when they say “nearby.” Mark only what helps someone make a decision.
| Official source | What it covers | URL / phone / radio | Backup way to reach it |
|---|---|---|---|
Opt-in people, skills, and resources
Record what someone wants others to know. Review it regularly and remove information when asked.
| Name / household | Check-in method | Help requested | Skill or resource offered | Limits / notes | Consent checked |

Status, needs, and offers
Start a fresh sheet for each operational period. Time-stamp updates and cross out old information without erasing it.
| Time | Location / person | Observed status or need | Offer / action | Source | Priority | Next check |
|---|---|---|---|---|---|---|
Tasks, messages, and handoffs
Give each task an owner and return time. Give each message a destination and delivery status.
| Time | Task or message | Owner / messenger | Destination | Return / follow-up | Status and outcome |
